How many wishes do people get on their birthday?

My dreams are yet to be fulfilled, my aspirations are pending.There is no point in celebrating it.
And both are my college seniors .
Well, I literally don't like receiving birthday wishes and it is the fact that I even don't like celebrating it.
Do countries with free health care generally have co-pays or deductibles to discourage use?
